In the overall planning, the service will be assisted with related medical information systems to facilitate personalized medical care planning, disposition, medicine allocations, etc. Following hospital discharge, with the assistance of telecare-related equipment, patients will receive long-term follow-up and continuous care under the guidance of case managers, registered nurses, and social workers who provide care and suggestions on health promotion to change the patients' lifestyles, in order to accomplish prevention before and proper management afterwards, and facilitate homecare. Meanwhile, the mechanism to compile multiple resources integrates personal insurance-related services, mutual support from similar patient groups, cooperative gyms, health foods and beverages, and other partners (Strategies 2 and 3) to provide a better health promotional care. The PTC platform plays the role of data filter and verifier. Verified data will be uploaded to cloud computing services (Strategy 4).
